4 | Date of birth (day-month-year) | Write down your birthday. The format should be day/month/year., for example, 12/01/1999. |
5 | Place of birth | Write down the city or town where you were born. |
6 | Country of birth | Write down the country you were born in. |
7 | Current nationality | Write your current nationality as it appears on your travel document. |
*Family members of EU, EEA, and Swiss citizens do not need to fill out the fields marked with asterisk (*).
