Transcripts should be sent directly from the school to DOPL or sealed in an envelope bearing the school’s stamp/seal on the envelope flap. Transcripts can be emailed to b1@utah.gov through a secure clearinghouse.

Inactive status is available if the license is active and in good standing. You must apply for this status by application and pay the applicable processing fees. It is important to note that inactive licenses, the license holder must continue to pay renewal fees.

However, if you will be performing any action or utilizing tools that are defined in the Dental and Dental Hygienist Practice Act or any other Utah Professional Practice Act(s) you will need a license in that profession.
