Transcripts should be sent directly from the school to DOPL or sealed in an envelope bearing the school’s stamp/seal on the envelope flap. Transcripts can be emailed to b1@utah.gov through a secure clearinghouse.

Can I start working once my license is active?

No. It is considered unprofessional conduct to engage in practice as a dentist or dental hygienist without prominently displaying a copy of the current Utah license.

Are dental assistants licensed in Utah?

Dental Assisting is not a licensed profession in the state of Utah.

What do I need to do if I do not want to renew my license?

A licensee is not required to inform the division if they choose to allow the license to expire.
Inactive status is available if the license is active and in good standing. You must apply for this status by application and pay the applicable processing fees. It is important to note that inactive licenses, the license holder must continue to pay renewal fees.

Do I qualify for application by endorsement?

You must have a current license in good standing and have been practicing for five consecutive years immediately preceding the submission of the Utah application.

As a Dental Hygienist, can I do a working interview once I submit my application?

Do I need to be licensed to provide/administer teeth whitening in the state of Utah?

The Division Statute and Rule does not directly address teeth whitening. However, if you will be performing any action or utilizing tools that are defined in the Dental and Dental Hygienist Practice Act or any other Utah Professional Practice Act(s) you will need a license in that profession.
